Lead training and onboarding for clinical coordinators supporting chronic care management and post-acute care programs, with a focus on CCM, PCM, BHI, and CoCM workflows. The role centers on education, chart review, and compliance support for residents in skilled nursing and long-term care settings. It also includes collaboration with leadership, preceptors, and interdisciplinary presenters to roll out documentation tools, process changes, and regulatory updates.
Key Responsibilities
- Lead new hire orientations and onboarding for clinical coordinators across multiple care management programs.
- Develop and deliver training plans, refresher education, and workflow updates for staff.
- Review charts and complete audits to support documentation quality, compliance, and CMS deadlines.
- Coordinate remote trainers, preceptors, guest speakers, and subject matter experts.
- Lead education on new EMR tools, documentation templates, and process rollouts.
- Support leadership with corrective action related to documentation, performance, or compliance gaps.
- Participate in interviewing, selection, and onboarding of new clinical coordinators.
- Track regulatory, billing, and policy changes and communicate updates to the team.
Required
- Registered Nurse or LPN license in good standing.
- At least 3 years of experience in clinical coordination, post-acute care, or quality assurance.
- Strong understanding of Medicare programs including CCM, PCM, BHI, and CoCM.
- Experience using EMR systems.
- Strong communication, teaching, documentation, and organizational skills.
Preferred
- Prior experience in education, training, or leadership.
- Experience with EMR platforms such as PCC or MatrixCare.
- Background in managing multiple training schedules and reporting workflows.
