Provide intake and follow-up psychiatric care for a diverse adult and family population, with a focus on evidence-based mental health treatment, psychiatric evaluations, and medication management. This role also includes coordinating referrals to therapy and coaching services as part of a connected care model, with emphasis on culturally responsive care.
Key Responsibilities
- Conduct psychiatric intake assessments and regular follow-up visits.
- Provide evidence-based medication management for mental health conditions.
- Evaluate patients’ psychiatric needs and adjust treatment plans as appropriate.
- Coordinate referrals to therapy and coaching services alongside prescribing care.
- Support whole-person, whole-family mental health care for patients from diverse backgrounds.
- Use the provider platform and matching technology to support care delivery and scheduling workflows.
Required
- Licensed Nurse Practitioner (NP) or certified Physician Assistant (PA-C).
- Experience or strong interest in mental health care.
- Ability to provide care in a contract role.
Preferred
- Experience delivering culturally responsive care.
- Interest in working within a connected, technology-enabled provider network.
- Prior experience with psychiatric evaluation and medication management.
