This role provides fully remote, video-based comprehensive primary care for patients needing acute, preventive, and chronic care support, with some pediatric and mental health visits included. The clinician also helps manage physician inboxes, prescribe medications and refills, and educate patients on care plans and treatment options. It is a telemedicine-focused role supported by a 24/7 operations and technical team, with a strong emphasis on patient outcomes and collaborative care.
Key Responsibilities
- Provide video-based comprehensive care for acute, preventive, and chronic conditions
- Support pediatric and mental health care within a broad primary care scope
- Manage Epic inbox tasks such as lab results, imaging follow-up, and patient questions
- Prescribe medications and refills as clinically appropriate
- Educate patients on treatment options, care plans, and self-management
- Collaborate with care team members and escalate issues when needed
- Deliver compassionate remote care while maintaining strong patient rapport
- Work weekend day shifts and one weekday shift on a regular schedule
Required
- Current board certification as a Family Nurse Practitioner
- Active licensure in Florida, New Jersey, New York, and/or Massachusetts, or an active NLC
- At least 5 years of clinical practice experience as a Family Nurse Practitioner in primary care
- At least 1 year of experience using Epic EMR
- Current enrollment in, or eligibility for enrollment as, a Medicare provider
- Experience treating both acute and chronic care conditions

- Ability to work weekend hours consistently
