This clinician provides fully remote primary care visits for adults and may also manage pediatric and mental health concerns as part of a broad family medicine or internal medicine scope. Care includes preventive, acute, and chronic condition management, with overnight coverage for a panel of patients across partner clinics. The role emphasizes longitudinal primary care, post-discharge follow-up, and population health work, supported by virtual workflows and clinical AI tools.
Key Responsibilities
- Provide preventive, acute, and chronic primary care in a fully remote, video-based setting.
- Manage a dedicated patient panel as the primary care physician.
- Refer patients to specialists or higher-acuity care when clinically indicated.
- Support population health activities such as annual wellness visits, care gap closure, transitions of care, and risk capture.
- Provide post-discharge follow-up to reinforce treatment plans and reduce readmissions.
- Practice within clinical guidelines and continuously evaluate care quality.
- Work overnight coverage shifts for partnership clinics.
- Use virtual care technology and EMR workflows to deliver efficient care.
Required
- Board certification in Family Medicine or Internal Medicine.
- Active licensure in New Jersey, Connecticut, or Massachusetts, or willingness to obtain these licenses.
- At least 3 years of post-residency experience.
- At least 2 years of experience in an in-person primary care setting.
- Experience with virtual visits.
- Prior experience using Epic EMR.
- Clean background and medical malpractice history.
- Willingness to work overnight 12-hour shifts.
Preferred
- Spanish language fluency.
- Experience managing a high-volume virtual primary care workflow.
