Provide virtual cardiovascular care for patients with atrial fibrillation, vascular disease, heart failure, and other heart conditions through televisits and same-day diagnostic reads. This role focuses on connected, tech-enabled care coordination, with work centered in a virtual clinic serving patients who need timely cardiology assessment and follow-up.
Key Responsibilities
- Conduct virtual cardiology visits for patients with a range of cardiovascular conditions.
- Provide same-day diagnostic reads as part of the clinical workflow.
- Coordinate care across a connected provider ecosystem to support timely treatment and follow-up.
- Use telemedicine tools and virtual care programs to manage ongoing cardiac care.
- Participate in care for patients with atrial fibrillation, vascular disease, and heart failure.
