Provide virtual primary care for a diverse patient panel, with a focus on preventive care, chronic disease management, and urgent care concerns. You will conduct video visits, support continuity between appointments through secure messaging, and build ongoing relationships with patients over time. The role is supported by a digital care platform and includes collaboration with experienced physicians in a relationship-based practice model.

Key Responsibilities

Conduct initial and follow-up primary care visits via video for adult patients.
Evaluate and manage chronic conditions such as diabetes, hypertension, and obesity.
Provide urgent care guidance and recommend in-person evaluation when clinically necessary.
Prescribe medications electronically when appropriate, excluding DEA and state-scheduled drugs.
Order, review, and follow up on laboratory testing.
Document visits with clear SOAP notes and follow-up recommendations.
Provide patient education, preventive care counseling, and lifestyle guidance.
Participate in peer review and quality-improvement initiatives.

Required

MD, DO, or equivalent degree.
Board certification in Family Medicine or Internal Medicine.
Active, unrestricted U.S. medical license in good standing.
Current licensure in NJ, FL, TX, or GA, or multiple active state licenses.
At least 4 years of post-residency clinical practice in the U.S.

Preferred

Experience managing chronic disease in a primary care setting.
Experience with virtual care or remote clinical workflows.
Strong comfort with digital documentation and telemedicine tools.
