Provide telehealth visits for older adults with chronic musculoskeletal and bone health needs, including intake assessments, safety review, medication support when appropriate, and ongoing physician oversight as patients progress through a PT-led care program. The role centers on using medical judgment to support function, fall prevention, and safe management of osteoporosis and related chronic conditions, rather than leading the long-term exercise plan. You will collaborate closely with the PT team and clinical leadership in a remote, digitally delivered model.
Key Responsibilities
- Conduct telehealth intake visits for older adults with musculoskeletal and bone health concerns.
- Provide safety guardrails and clinical oversight within a PT-led care model.
- Refer patients into the ongoing PT program and support continuity of care.
- Manage medications when appropriate, including bone health-related treatment decisions.
- Use clinical judgment to reinforce education on diet, movement, fall prevention, and functional aging.
- Coordinate asynchronously with PTs and clinical leadership on patient progress and care planning.
- Document clearly and efficiently in a digital care workflow.
Required
- MD or DO degree.
- Active California medical license.
- At least 5 years of clinical experience after residency.
- Background in Internal Medicine, Family Medicine, Geriatrics, or Endocrinology preferred as a core fit.
Preferred
- Experience in telehealth, primary care, geriatrics, or digital health.
- Experience managing osteoporosis, bone health, or older adult chronic disease.
- Multi-state licensure or IMLC eligibility.
