Provide virtual adult primary care through asynchronous messaging and video visits, managing a broad mix of preventive, acute, and chronic concerns across varying levels of complexity. Clinicians maintain continuity of care by triaging evolving symptoms, ordering and interpreting labs and imaging, and guiding patients to the next appropriate step rather than handling single, transactional encounters. The role is supported by a tech-enabled care model with physician collaboration, specialist e-consults, and operations support to reduce administrative work.
Key Responsibilities
- Provide asynchronous text-based and synchronous video-based care for adult patients.
- Conduct broad-scope primary care across preventive, acute, urgent, and chronic conditions.
- Perform clinical triage and diagnostic synthesis for unclear or evolving patient concerns.
- Order and interpret labs and imaging to support evidence-based decision-making.
- Coordinate rapid specialist e-consults and collaborate with physicians and specialists as needed.
- Guide patients to appropriate in-person care when virtual care is not sufficient.
- Maintain clinical ownership, continuity, and follow-through across the care journey.
- Contribute to workflow development, policies, and quality improvement efforts.
Required
- Advanced Practice Nurse Practitioner with a background in Family Medicine.
- At least 6,240 clinical practice hours, or roughly 3 years of full-time experience.
- Experience in outpatient or ambulatory primary care with adult medicine exposure.
- Active, unrestricted NP licensure covering at least 40% of the U.S. population.
- Eligibility to see Medicare and Medicaid patients.
- Comfort working independently within scope and collaborating with physicians and specialists.
- Ability to work effectively in a virtual-first environment.
- Clean practice record with no history of malpractice.
Preferred
- Prior telehealth experience.
- Experience overseeing clinical groups, workflows, or policies.
- Multi-state licensure in priority states such as California, Texas, Florida, or New York.
- Comfort with clinical triage, diagnostic ambiguity, and rapid workflow adaptation.
