This RN role focuses on in-person and phone-based triage for acutely ill patients, with patient education around chronic conditions such as diabetes and high blood pressure. The nurse also helps keep preventive care on track, including screenings and routine tests, while supporting referrals, prior authorizations, and in-office procedures. It is a full-time, clinic-based position that works closely with physicians in a collaborative, team-based care model.
Key Responsibilities
- Conduct in-person or phone triage for acutely ill patients.
- Take vital signs, document health histories, and update the EMR.
- Provide education on chronic disease management, medications, treatments, and lifestyle changes.
- Coordinate preventive care, routine check-ups, screenings, and other recommended tests.
- Handle prior authorizations and coordinate referrals.
- Assist physicians with exams and in-office procedures.
- Perform phlebotomy, administer vaccines, and give injections.
Required
- Active RN license.
- At least 2 years of clinical triage experience.
- Ability to work full-time in an in-office clinic setting.
- Reliable access to personal transportation.
Preferred
- Pediatric experience.
- Acute care or inpatient experience.
- Bilingual English/Spanish communication skills.
