This clinician will provide hybrid primary care for adults with preventive needs and chronic conditions, including diabetes, hypertension, obesity, and mental health concerns. Care is delivered both virtually and in person, with monthly travel to onsite employer clinics and follow-up care for those patients through a digital platform. The role emphasizes shared decision-making, culturally sensitive care, and use of technology in a team-based practice.
Key Responsibilities
- Provide primary care for preventive health needs and chronic illness management.
- Conduct virtual follow-up visits for onsite clinic patients and patients across the national virtual network.
- Provide in-person care during monthly onsite clinic travel blocks.
- Manage complex chronic conditions such as diabetes, hypertension, obesity, and mental health concerns.
- Use a proprietary clinician-patient platform and other digital tools to deliver care.
- Collaborate with team members to solve patient problems efficiently and support coordinated care.
- Communicate complex medical information clearly and support shared decision-making.
Required
- MD or DO with active board certification in Family Medicine or Internal Medicine.
- Active Texas medical licensure, or ability to obtain licensure by the start date.
- At least 5 years of clinical experience in primary care and chronic care.
- Comfort with basic procedural skills.
- Strong communication skills, including the ability to explain complex ideas in simple language.
Preferred
- Experience with digital-first or virtual medicine.
- Interest in innovative care models that combine evidence-based practice with creative approaches.
- High comfort using technology and applications for patient care.
- Professional fluency in English and Spanish, including cultural nuance for Spanish-speaking patients.
