This physician provides in-clinic and digital primary care for patients across the lifespan, with emphasis on preventive care, chronic disease management, urgent care, and pediatrics. The role supports a collaborative, technology-enabled practice that uses shared decision-making and individualized treatment planning. The clinician also works closely with behavioral health, virtual triage, and subspecialty partners to address complex medical and social needs.
Key Responsibilities
- Provide primary care visits in the clinic and through digital channels
- Deliver preventive health care, chronic disease management, urgent care, and pediatric care
- Collaborate with behavioral health, virtual urgent care and triage, and clinical subspecialists
- Educate patients about health conditions and treatment options
- Develop individualized care plans with patients
- Use data, technology, and operational processes to support patient outcomes
- Coordinate care for patients with complex clinical and social needs
Required
- Board certification in Family Medicine
- Active Texas medical licensure or ability to obtain licensure by start date
- At least 5 years of clinical experience in primary care and chronic care across ages
- Comfort providing care for pediatric patients
- Comfort with basic procedural skills
Preferred
- Bi-lingual English and Spanish
- Experience working in a technology-enabled practice
- Strong ability to translate complex medical information into simple language
