Provide phone- and video-based follow-up care for patients recently discharged from the hospital, with a focus on medication reconciliation, symptom screening, and arranging needed labs, medications, or supplies. This role also involves telephonic outreach, care coordination across hospital, ED, pharmacy, and community settings, and helping patients understand and engage with a digital-first care model. The work is population-health oriented and supports underserved patients with complex medical needs.
Key Responsibilities
- Conduct telephonic outreach and scheduled video/phone visits for post-hospital discharge patients.
- Provide transition-of-care follow-up focused on medication reconciliation and symptom screening.
- Order follow-up labs, medications, and supplies as needed.
- Coordinate care across hospitals, emergency departments, pharmacies, and community providers.
- Collaborate with nursing and administrative team members.
- Build patient trust and explain Galileo’s care model during follow-up outreach.
- Contribute to individual performance targets and population health goals.
Required
- Certified Nurse Practitioner in Adult, Family, or Geriatrics.
- Active Illinois state licensure.
- Ability to obtain additional state licensure as needed.
- 5+ years of post-graduate clinical experience in primary care or geriatrics.
- Experience caring for Medicare and Medicaid patients with complex, comorbid conditions.
Preferred
- Experience in a skilled nursing facility or long-term care facility.
- Motivational interviewing skills.
- Experience with virtual care delivery in a digital-first environment.
- Familiarity with Google Suite and Slack.
- Bilingual English/Spanish skills with cultural fluency.
- Active licensure in additional states, including TX, PA, NJ, NY, MA, MN, LA, and/or SC.
