This Nurse Practitioner role focuses on phone and video-based transitional care for patients recently discharged from the hospital, with an emphasis on medication reconciliation, symptom screening, and arranging follow-up needs. The work centers on underserved patients and those with complex medical needs, while also involving care coordination across hospitals, emergency departments, pharmacies, and community providers. It is a digital-first, team-based setting with collaboration across nursing and administrative colleagues.
Key Responsibilities
- Conduct telephonic outreach and scheduled video or phone visits for post-discharge patients.
- Provide follow-up care through medication reconciliation, symptom screening, and ordering needed labs, medications, or supplies.
- Coordinate care across hospital, ED, pharmacy, and community settings to support smooth transitions.
- Build patient trust, explain the care model, and connect patients to appropriate services.
- Collaborate with nursing team members and administrative colleagues.
- Use performance targets and population health goals to guide day-to-day work.
Required
- Certified Nurse Practitioner in Adult, Family, or Geriatrics.
- Active Connecticut state licensure.
- Minimum 5 years of post-graduate clinical experience in primary care or geriatrics.
- Experience with Medicare and Medicaid populations managing complex, comorbid conditions.
- Ability to work a minimum of 24 hours per week.
- Comfort working in a digital-first or virtual care environment.
Preferred
- Additional active state licenses, especially NJ, TX, PA, IL, NY, MA, MN, LA, or SC.
- Experience in a skilled-nursing facility or long-term care facility.
- Motivational interviewing skills.
- Familiarity with Google Suite and Slack.
- Bilingual English and Spanish skills with cultural fluency.
