This clinician will provide hybrid primary care for patients across the lifespan, with a strong focus on preventive care and chronic disease management such as diabetes, hypertension, obesity, and mental health. Care is delivered both virtually and in person, including monthly travel to employer onsite clinics, with follow-up and ongoing visits handled through a digital platform. The role emphasizes shared decision-making, culturally responsive care, and collaboration within a team-based, tech-enabled practice.
Key Responsibilities
- Provide primary care visits for patients from age 0-100, including preventive care and chronic illness management.
- Deliver virtual follow-up care and conduct in-person visits during scheduled onsite clinic travel.
- Manage common chronic conditions such as diabetes, hypertension, obesity, and mental health concerns.
- Use a proprietary clinician-patient platform and related tools to document and coordinate care.
- Collaborate with multidisciplinary team members to solve patient problems efficiently.
- Provide compassionate, culturally sensitive care with shared decision-making.
- Perform basic procedural tasks as needed in primary care practice.
Required
- Active board certification in Family Medicine.
- Active Texas medical licensure, or ability to obtain it by the start date.
- At least 5 years of clinical experience in primary care and chronic care across all ages.
- Comfort with basic procedural skills.
- Strong communication skills and ability to explain complex ideas clearly.
Preferred
- Experience with or interest in digital-first healthcare and virtual medicine.
- Interest in innovative care models that combine evidence-based practice with creative approaches.
- Professional fluency in English and Spanish, including cultural nuance for native Spanish speakers.
- High comfort using technology and common digital collaboration tools.
