Provide virtual primary care through asynchronous text-based chat and synchronous video, with a strong focus on complex and chronic conditions such as diabetes, hypertension, obesity, and mental health concerns. Care is broad across the lifespan and delivered in a digital-first, team-based model that emphasizes evidence-based practice, shared decision-making, and culturally sensitive communication. The role includes efficient problem-solving with support from a collaborative clinical team and use of a proprietary patient platform.
Key Responsibilities
- Conduct virtual primary care visits by text chat and video.
- Provide care for patients across the lifespan, including complex and chronic conditions.
- Manage common chronic illnesses such as diabetes, hypertension, obesity, and mental health concerns.
- Use a proprietary clinician-patient platform and other digital tools to deliver care.
- Collaborate with clinical team members to coordinate care and solve patient problems efficiently.
- Provide compassionate, patient-centered care with shared decision-making.
- Work scheduled evening and weekend shifts as part of a 24/7 care model.
Required
- Certified Nurse Practitioner.
- Active Texas state licensure.
- Ability to obtain additional state licensures.
- Medical Spanish fluency, verbal and written.
- At least 5 years of advanced clinical experience in complex care, chronic care management, or primary care.
Preferred
- Experience in digital-first healthcare or virtual medicine.
- Experience with innovative care models that combine evidence-based practice with creative problem-solving.
- Comfort using technology and multiple applications to deliver care virtually.
