Provide telemedicine primary care and tele-triage for adults in post-acute and long-term care settings, with an emphasis on geriatric, family, and internal medicine concerns. The work includes coordinating with facility staff, family members, and other clinicians, reviewing consent and treatment plans, and managing same-day documentation and follow-up. This role relies on video-based visits, established site protocols, and collaborative care across an interdisciplinary support team.
Key Responsibilities
- Provide primary-care telemedicine visits for adult patients in skilled nursing, assisted living, independent living, and home settings.
- Conduct tele-triage calls according to established protocols and on-call scheduling.
- Coordinate with family members, facility staff, administration, and specialty providers as needed.
- Confirm treatment and telehealth consents before visits and ensure required documentation is completed the same day.
- Review visit plans with the patient’s regular provider through the EMR.
- Manage medication-related issues, including narcotic prescriptions under policy guidance.
- Use telemedicine equipment, delivery platforms, and software to support remote care.
- Support quality, billing, CCM, and other administrative tasks related to telehealth operations.
Required
- Master’s degree in Nursing and a current, valid advanced practice nursing license in the state of practice, or completion of an accredited physician assistant program.
- National certification as a Physician Assistant, if applying as a PA.
- Current and valid certifications needed to perform the role.
- Licensed in Kentucky and Indiana to start.
- Ability to maintain multiple state licenses as requested.
Preferred
- Specialization in geriatric medicine, family medicine, or internal medicine.
