Provide telemedicine primary care and triage for adults in post-acute and long-term care settings, including skilled nursing, assisted living, independent living, and patients in their homes. Day-to-day work centers on family medicine, geriatric medicine, or internal medicine concerns, with consults and communication that may include family members, facility staff, and other providers. The role emphasizes same-day documentation, protocol-based care, and coordination with facility partners using telehealth equipment and EMR workflows.
Key Responsibilities
- Conduct telemedicine visits and on-call tele-triage for adult patients across contracted care sites.
- Provide primary care assessment, advice, intervention, and referral within family, geriatric, or internal medicine practice standards.
- Coordinate with facility staff, administration, family members, and specialty providers as needed.
- Confirm treatment and telehealth consent before visits and ensure required documentation is completed the same day.
- Manage patient follow-up, next-visit planning, CCM, billing, and communication with the patient’s regular provider.
- Use telemedicine equipment, videoconferencing platforms, software, and EMR tools to deliver care and document orders.
- Follow prescribing policies, including narcotic prescription requirements, and adhere to site-specific triage protocols.
- Participate in quality, safety, confidentiality, and regulatory compliance practices.
Required
- Master’s degree in Nursing from an accredited program and a current, valid APRN license in the state of practice, or graduation from an accredited physician assistant program with national certification.
- Current and valid certifications required to perform the role.
- Active licensure in Kentucky and Indiana to start.
- Ability to obtain additional state licenses if requested.
Preferred
- Specialization in geriatric, family, or internal medicine.
