This physician reviews outpatient medical records to validate documented diagnoses, support accurate risk adjustment, and identify clinical documentation opportunities. The work is fully remote and non-patient-facing, with a focus on independent chart review, documentation queries, and evidence-based diagnosis validation. It also involves collaboration with the medical team to improve documentation quality and accuracy using Kaiser Permanente’s secure review platform.
Key Responsibilities
- Review outpatient medical records in a secure online documentation review platform.
- Evaluate documented diagnoses against clinical evidence and medical records.
- Perform diagnosis validation and identify opportunities for improved documentation.
- Submit clear documentation queries and recommendations to the medical team.
- Apply medical knowledge, clinical guidelines, and evidence-based practices to chart review.
- Conduct independent clinical research when needed to support review decisions.
- Complete required training and follow documentation standards consistently.
- Meet productivity and quality expectations while working independently in a remote environment.
Required
- MD, DO, or MBBS degree.
- Strong clinical knowledge and sound medical judgment.
- Excellent written and verbal communication skills.
- Strong understanding of medical terminology.
- Ability to analyze complex medical records and documentation.
- Strong computer proficiency with EMR/EHR experience.
Preferred
- Experience in medical risk adjustment or HCC.
- Experience with clinical documentation improvement (CDI) or ICD-10 diagnosis review.
- Outpatient clinical experience.
- Background in internal medicine, family medicine, primary care, hospital medicine, general medicine, or geriatrics.
- Experience in utilization review, physician advisory, medical review, clinical quality, population health, or value-based care.
- ECFMG certification for foreign-trained physicians.
