Provide virtual primary care for patients in a 24/7/365 clinic setting, focusing on common primary care needs and continuity of care during bridge shifts. This role is part of a tech-enabled model that uses clinical expertise alongside AI to improve access, affordability, and effectiveness of care. The schedule is full-time and includes weekday evening coverage with a rotating weekend shift.
Key Responsibilities
- Provide virtual primary care visits for a broad range of patient needs
- Assess common acute and chronic primary care concerns
- Coordinate care within a 24/7/365 clinic workflow
- Use technology-enabled tools to support efficient clinical decision-making
- Collaborate with clinical team members to maintain continuity of care across shifts
- Document patient encounters and care plans in the virtual care platform
Required
- Active Nurse Practitioner credentialing/licensure for clinical practice
- Experience practicing in primary care or a closely related outpatient setting
- Availability to work full-time bridge-shift hours, including a rotating weekend shift
