This role focuses on remote review of outpatient medical records to confirm whether assigned diagnoses are supported by documented clinical evidence. The reviewer evaluates findings, medications, labs, testing, and other chart details, then writes concise comments for treating teams when clarification is needed. Work is long-term, fully remote, and centered on accurate, guideline-based diagnosis validation in a quality-reviewed environment.
Key Responsibilities
- Review patient records for assigned diagnoses.
- Evaluate clinical findings, medications, labs, testing, and supporting documentation.
- Determine whether diagnoses are clinically supported or require clarification.
- Prepare clear, concise clinical comments for review by treating medical teams.
- Enter findings accurately into an online review platform.
- Apply internal guidelines, independent research, and quality feedback.
- Maintain productivity, accuracy, and confidentiality standards.
Required
- Completed medical school education.
- Strong clinical knowledge and medical terminology.
- Excellent written communication skills.
- Ability to independently review complex medical records.
- Ability to learn new workflows and apply feedback.
- Current residence within the United States.
- Full-time availability during Pacific Time business hours.
Preferred
- Risk adjustment or HCC experience.
- Diagnosis validation or medical chart review experience.
- CDI, coding, ICD-10, or clinical documentation experience.
- Outpatient or ambulatory experience.
- ECFMG certification for foreign-trained physicians.
