Provide telehealth-based care for adults with endocrine and metabolic health needs, with a strong focus on weight management and diabetes. The role centers on structured synchronous and asynchronous visits, medication management, and ongoing clinical decision-making for chronic disease care. This is also a clinical lead position, with responsibility for shaping protocols, workflows, and care pathways as the practice grows.
Key Responsibilities
- Conduct telehealth visits for weight management, diabetes, and related endocrine/metabolic conditions.
- Provide medication management, including titration of GLP-1 therapies, insulin, and other related treatments.
- Develop and refine clinical protocols for glycemic management, side-effect triage, dose escalation, and treatment transitions.
- Design workflows for intake, visit structure, documentation, prescribing, and follow-up in collaboration with technology and operations teams.
- Collaborate with an endocrinologist advisor on complex cases and protocol refinement.
- Document clinical decision logic, escalation criteria, and care pathways to support future scaling and systematization.
- Participate in weekly operational reviews and provide feedback on workflow friction and patient experience.
- Contribute clinical content for structured reports shared with referring physicians.
Required
- Active Nurse Practitioner license in New York State.
- Family, adult-gerontology, or acute care NP background.
- Experience caring for patients with diabetes, obesity, or metabolic conditions.
- Experience prescribing and titrating GLP-1 therapies, insulin, or related medications.
- Background in adult or geriatric care settings such as internal medicine or primary care.
- Current payer credentialing in New York or an active CAQH profile for enrollment.
Preferred
- Multistate licensure or NLC compact enrollment covering additional states.
- Telehealth or virtual care experience.
- Board certification in obesity medicine, diabetes care and education, or a related endocrinology-adjacent specialty.
- Experience with Athena EHR.
- Experience with remote patient monitoring workflows.
- Leadership, training, or team development experience.
