Provide brief, structured telehealth intake visits for seniors, focusing on holistic assessment, high-risk conditions, and unmet social needs that affect long-term well-being and independence. The role emphasizes care navigation and oversight rather than primary diagnosis or prescribing, with patients enrolled into ongoing advocacy support. You will collaborate closely with RN Health Advocates and use a tech-enabled platform to document findings, support individualized care plans, and help refine the care model.
Key Responsibilities
- Conduct brief telehealth E/M intake visits to establish care and assess patient needs.
- Evaluate seniors for high-risk medical issues, social determinants of health, and barriers to care.
- Validate and oversee individualized care plans with RN Health Advocates.
- Document assessments, diagnoses, risk factors, and SDOH Z-codes in the platform.
- Provide clinical guidance to advocates managing complex patient cases.
- Collaborate with operations and technology teams to improve workflows and AI-enabled tools.
Required
- MD or DO degree.
- Active, unrestricted California medical license.
- Physical presence in California.
- Active PTAN/PECOS enrollment with Medicare.
- Familiarity with Medicare programs and SDOH documentation.
- Willingness to obtain additional state medical licenses as needed.
Preferred
- Prior telehealth experience.
- Board certification in Internal Medicine, Family Medicine, or Geriatrics.
- Multistate licensure through an IMLC state.
- Fluency in additional languages.
