This RN role supports seniors with complex medical and social needs through remote video and phone-based advocacy. The clinician helps patients understand their care, manage chronic conditions, and navigate barriers such as transportation, food insecurity, insurance issues, and access to primary and specialty care. The work is highly collaborative with physicians and centers on long-term relationship building, care coordination, and practical problem-solving.
Key Responsibilities
- Guide seniors through complex medical and insurance processes in plain language.
- Coordinate care across physicians, clinics, insurance companies, and other providers.
- Build individualized care plans that address both clinical needs and social determinants of health.
- Identify and work through barriers such as denied claims, missed referrals, unaffordable medications, and lack of a primary care provider.
- Coach patients on health literacy and self-management for chronic conditions such as diabetes, chronic pain, mobility issues, and medication adherence.
- Lead difficult conversations with empathy and firm boundaries.
- Use new EHR and AI-enabled tools to support advocacy and documentation workflows.
- Share frontline insights to improve care delivery and operational processes.
Required
- Active California Registered Nurse (RN) license in good standing.
- Based in the United States.
- Experience working with diverse, vulnerable patient populations.
- Strong interpersonal, listening, and communication skills.
- Strong organizational and administrative skills.
- Comfort learning a new EHR and evolving operational tools and processes.
Preferred
- Background in community health or health coaching.
- Experience with Medicare populations.
- Fluency in Spanish or another language.
- Compact RN license.
