Provide virtual nursing support for Medicare Advantage members during Annual Wellness Visits, with a focus on chart preparation, review of medical history and medications, and identification of preventive care gaps. Work is fully remote and conducted through telehealth visits, with responsibilities centered on helping providers deliver compliant, patient-centered AWVs. The role also includes documentation, quality support, and coordination across multiple virtual care platforms in a structured home-office setting.
Key Responsibilities
- Prepare AWV charts to support provider efficiency and CMS compliance.
- Review and summarize medical, surgical, and family histories with patients during virtual visits.
- Document medication lists, adherence, preventive screenings, immunizations, and assessment findings.
- Identify overdue screenings, risk factors, and care gaps before visits.
- Communicate concise patient summaries and key findings to providers.
- Coordinate with patients to obtain missing or clarified information as needed.
- Maintain accurate, timely, and confidential documentation in EHR and telehealth systems.
- Support quality, patient engagement, and satisfaction outcomes while following HIPAA and CMS standards.
Required
- Active, unrestricted RN or LPN/LVN license in a compact state.
- At least 3 years of clinical nursing experience.
- Experience in a high-volume virtual or telehealth clinic environment.
- Proficiency with EHR systems and teleconferencing platforms.
- Dedicated, private home workspace for confidential patient interactions.
- Strong communication skills and ability to work independently in a remote setting.

- Experience with Medicare Annual Wellness Visits.
