Provide scheduled and on-demand virtual visits for Medicare Advantage members, with a focus on Annual Wellness Visits, E/M visits, new patient evaluations, follow-ups, chronic condition management, and medication review. Care is delivered by phone or video in a fully remote setting, with support for remote monitoring and care-gap closure. The role includes accurate EHR documentation, adherence to telehealth and CMS requirements, and participation in remote training and workflow updates.
Key Responsibilities
- Conduct Annual Wellness Visits and E/M virtual visits.
- Perform new patient evaluations and follow-up consultations.
- Manage chronic conditions and support remote monitoring.
- Assess patient conditions and review medications.
- Provide evidence-based care recommendations during virtual consultations.
- Build patient rapport through effective communication in a telehealth setting.
- Accurately document encounters in the EHR.
- Maintain compliance with telehealth and CMS requirements.
- Participate in remote training sessions and workflow updates.
Required
- Board-certified MD or DO, or an experienced Nurse Practitioner.
- Active, unrestricted license in multiple states.
- Medicare participating physician status, or willingness to become one.
- Strong documentation and patient communication skills.
Preferred
- Telehealth experience.
