Provide virtual functional medicine care for patients with complex chronic illness, including mold-related illness, tick-borne infections, autoimmune conditions, hormonal concerns, gut and adrenal imbalances, and longevity/healthspan goals. Care centers on comprehensive assessment, root-cause treatment planning, follow-up, and patient education, with many patients presenting with multiple overlapping conditions. The role also includes collaboration on protocols, case discussions, and use of digital tools to support efficient, personalized care.
Key Responsibilities
- Conduct comprehensive functional medicine assessments, diagnoses, treatment plans, and follow-up visits.
- Provide individualized, layered care plans that address root causes of complex chronic illness.
- Monitor patient progress and adjust treatment plans based on response and evolving needs.
- Educate patients on functional medicine principles and active participation in their care.
- Participate in clinician team calls, case discussions, and collaborative problem-solving.
- Support development and refinement of clinical protocols, best practices, and evidence-based guidelines.
- Use telemedicine platforms, AI research tools, AI scribe services, and other digital workflows to support care delivery.
- Contribute to patient education materials such as handouts, workshops, and programs.
Required
- Licensed Nurse Practitioner or Physician Assistant; California license required.
- MD or DO candidates may also be considered.
- Experience working in a medical practice, with functional and/or integrative medicine experience preferred.
- Ability to manage a virtual caseload independently in a fast-paced, remote setting.
- Comfort caring for patients with complex, multi-layered chronic health concerns.
- Reliable home office setup with quiet workspace, internet, and appropriate computer equipment.
Preferred
- Functional medicine background, including IFM and/or ADAPT certification.
- Experience with hormone optimization, immune support, mold-related illness, tick-borne disease, or longevity-focused care.
- Familiarity with functional medicine testing such as blood, saliva, stool, and urine studies.
- Multi-state licensure or willingness to expand licensure.
- Experience with Zoom, Google Suite, Google Drive, and AI-enabled clinical tools.
- Interest in mentoring or supporting fellowship-level clinicians.
