The clinician monitors patients with chronic cardiovascular and metabolic disease, reviewing vital signs, symptoms, and labs to identify those who need intervention. The role includes virtual follow-up visits, triage of abnormal readings, and support for medication adherence, treatment plan changes, and lifestyle goals. It is a remote, part-time weekend position that works closely with nurse practitioners and patients’ physicians through a tech-enabled remote monitoring workflow.
Key Responsibilities
- Monitor patient vitals, symptoms, and labs to identify patients needing clinical intervention.
- Follow up with patients who have abnormal readings to gather more clinical information and triage appropriately.
- Respond to inbound patient questions and escalations by phone and text message.
- Lead virtual follow-up visits to support program enrollment, treatment plan changes, medication adherence, and lifestyle goals.
- Support clinical documentation and prepare care summaries for patients’ physicians.
- Collaborate with Cadence Nurse Practitioners and patients’ physicians or providers to coordinate care.
- Support care for patients with CHF, hypertension, type 2 diabetes, and other chronic conditions.
Required
- Active Registered Nurse license with compact multistate licensure.
- At least 4 years of experience treating patients with CHF, hypertension, and diabetes in outpatient or inpatient settings.
- Experience working in a CHF bridge clinic environment.
- Experience using remote patient monitoring technology.
- Ability to monitor patient vitals, symptoms, and labs and triage abnormal findings.
Preferred
- Experience supporting care in a digital or tech-enabled chronic care model.
- Strong interest in patient experience and healthcare improvement through digital innovation.
- Comfort working independently in a remote setting.
