This role centers on brief telehealth intake visits for older adults to establish medical necessity for care navigation services, identify high-risk conditions, and surface unmet social needs. The physician also reviews and approves individualized care plans and documents risk factors, diagnoses, and SDOH codes in the platform. Work is fully remote and focused on collaboration with advocates and clinical operations in a tech-enabled care navigation model.
Key Responsibilities
- Conduct structured telehealth E/M visits or Annual Wellness Visits to assess eligibility for care navigation services.
- Identify high-risk medical conditions and unmet social needs during intake consultations.
- Document findings accurately in the platform, including diagnoses, risk factors, and SDOH Z-codes.
- Validate and approve individualized care plans aligned with the patient’s treatment plan and continued medical necessity.
- Collaborate with advocates and clinical operations to support safe and compliant enrollment.
- Use EMRs, care management platforms, and telehealth workflows to complete visit documentation and follow-up.
Required
- M.D. or D.O. degree.
- Active, unrestricted U.S. medical license.
- Bilingual Spanish proficiency.
- At least 2 years of experience in primary care, telehealth, or care management.
Preferred
- Broad multistate licensure, such as IMLC participation or 50-state coverage.
- Board certification in Internal Medicine, Family Medicine, or Geriatrics.
- Familiarity with Medicare programs, care navigation, incident-to requirements, and SDOH documentation.
- Experience with EMRs, care management platforms, and telehealth workflows.
