This physician role focuses on brief telehealth intake visits for older adults to establish medical necessity for care navigation services, identify high-risk conditions, and surface unmet social needs. The work centers on structured E/M visits or Annual Wellness Visits, with documentation in the platform to support eligibility, coding, and ongoing care planning. The clinician partners closely with advocates and clinical operations in a tech-enabled model that combines medical oversight with social support.
Key Responsibilities
- Conduct telehealth intake visits, including E/M visits or Annual Wellness Visits, to assess eligibility for care navigation services.
- Identify high-risk conditions, unmet social needs, diagnoses, and risk factors that support medical necessity.
- Document findings in the platform with accurate coding, SDOH Z-codes, and clinical details.
- Validate and approve individualized care plans aligned with the patient’s treatment plan.
- Coordinate with advocates and clinical operations to support safe, compliant enrollment.
- Use telehealth workflows, EMRs, and care management platforms to complete visits and documentation.
Required
- MD or DO degree with an active, unrestricted U.S. medical license.
- 2+ years of experience in primary care, telehealth, or care management.
- Bilingual Mandarin proficiency.
- Experience with telehealth workflows and care management platforms.
Preferred
- Broad multistate licensure, including IMLC participation or 50-state coverage.
- Board certification in Internal Medicine, Family Medicine, or Geriatrics.
- Familiarity with Medicare programs, care navigation, incident-to requirements, and SDOH documentation.
