Conduct brief telehealth intake visits for older adults to establish medical necessity for care navigation services, identify high-risk conditions and unmet social needs, and determine eligibility for ongoing support. These visits include structured E/M encounters or Annual Wellness Visits and are used to confirm alignment with the patient’s medical treatment plan. The role also involves working closely with care advocates and nurse care managers in a tech-enabled, remote care model.
Key Responsibilities
- Conduct telehealth intake visits to assess medical necessity, high-risk conditions, and social needs.
- Document clinical findings in the platform, including diagnoses, risk factors, and SDOH Z-codes.
- Validate individualized care plans and confirm continued medical necessity.
- Collaborate with care advocates and nurse care managers to align clinical and social support.
- Use EMRs, care management tools, and telehealth workflows to support intake and documentation.
Required
- Completion of an accredited Nurse Practitioner program.
- Active Nurse Practitioner or APRN license in good standing.
- Multi-state licensure.
- At least 2 years of experience in primary care, telehealth, or care management.
- Bilingual Mandarin proficiency.
Preferred
- Familiarity with Medicare programs, including care navigation and incident-to requirements.
- Experience documenting social determinants of health.
- Proficiency with EMRs and care management platforms.
