Perform brief telehealth intake visits for older adults to establish medical necessity for care navigation services, identify high-risk conditions and unmet social needs, and confirm eligibility for ongoing support. The work centers on structured E/M visits or Annual Wellness Visits, with documentation that supports appropriate coding, SDOH Z-codes, and care plan approval. You collaborate with care advocates and nurse care managers in a tech-enabled, integrated care model focused on safe, compliant enrollment into the right level of support.
Key Responsibilities
- Conduct brief telehealth intake visits to assess medical necessity for care navigation services.
- Identify high-risk conditions, unmet social needs, diagnoses, and risk factors during the initial assessment.
- Document visit findings in the platform with accurate coding and SDOH Z-code use.
- Develop and approve individualized care plans aligned with the patient’s medical treatment plan.
- Collaborate with care advocates and nurse care managers to finalize support plans.
- Ensure patients are enrolled safely, compliantly, and compassionately into the appropriate level of care.
Required
- Completion of an accredited Nurse Practitioner program.
- Active Nurse Practitioner or APRN license in good standing.
- Multistate licensure.
- At least 2 years of experience in primary care, telehealth, or care management.
Preferred
- Familiarity with Medicare programs, including care navigation and incident-to requirements.
- Experience documenting social determinants of health (SDOH).
- Proficiency with EMRs, care management platforms, and telehealth workflows.
- Bilingual ability.
