Provide comprehensive outpatient primary care for patients across the full lifespan, including preventive care and chronic disease management, in a value-based care setting. The role centers on managing an assigned patient panel and delivering patient-centered care with strong clinical and operational support. You will also collaborate with physicians, APPs, and care teams, with optional in-office procedures and shared after-hours call coverage.
Key Responsibilities
- Provide comprehensive primary care to patients across the full lifespan
- Manage an assigned patient panel, including preventive care and chronic disease management
- Deliver high-quality care aligned with value-based care principles
- Collaborate with physicians, APPs, and care teams to coordinate care and improve outcomes
- Participate in care coordination and quality improvement initiatives
- Use the EMR to document care and manage patient records
- Provide after-hours call coverage for your patient panel
- Incorporate in-office procedures based on experience and interest
Required
- MD or DO degree from an accredited medical school
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active, unrestricted Rhode Island medical license or eligibility to obtain one
- Commitment to patient-centered, value-based care
- Ability to manage a broad range of primary care conditions
