This physician will provide comprehensive outpatient primary care for patients across the full lifespan, managing preventive care, chronic disease, and a broad range of primary care conditions. The role is focused on value-based care in a team-based setting, with support from physicians, APPs, and care teams. In-office procedures may be incorporated based on experience and interest, and after-hours call coverage is part of the panel-based practice model.
Key Responsibilities
- Provide comprehensive primary care across the full lifespan in a 100% outpatient setting
- Manage an assigned patient panel, including preventive care and chronic disease management
- Deliver patient-centered care aligned with value-based care principles
- Collaborate with physicians, APPs, and care teams to coordinate care and improve outcomes
- Participate in care coordination and quality improvement initiatives
- Use the EMR to document care and manage patient records
- Provide after-hours call coverage for your patient panel, with a shared model may be available
- Incorporate in-office procedures based on experience and interest
Required
- MD or DO degree from an accredited medical school
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active, unrestricted Rhode Island medical license, or eligibility to obtain one
- Commitment to patient-centered, value-based care
- Strong clinical judgment and ability to manage a broad range of primary care conditions
- Excellent communication and collaboration skills
