Provide comprehensive primary care for a diverse patient panel across the full lifespan in a fully outpatient, value-based setting. This role focuses on preventive care, chronic disease management, and coordinated follow-up, with support from physicians and advanced practice providers in a team-based practice. The position also includes EMR documentation, quality improvement work, and the opportunity to perform in-office procedures based on experience and scope.
Key Responsibilities
- Provide comprehensive primary care to patients across the full lifespan
- See patients in a 100% outpatient setting
- Manage an assigned panel, including preventive care and chronic disease management
- Collaborate with physicians, advanced practice providers, and care teams to coordinate care and improve outcomes
- Practice with physician collaboration, including consultation and referral as appropriate
- Participate in care coordination and quality improvement initiatives
- Use the EMR to document care and manage patient records
- Provide after-hours call coverage for the patient panel
Required
- Graduate of an accredited Nurse Practitioner program
- Board certified as a Nurse Practitioner in a primary care specialty such as FNP or AGNP
- Licensed or eligible for APRN licensure in Rhode Island
- Active DEA registration or eligibility to obtain it
- Rhode Island Controlled Substances Registration or eligibility to obtain it
- Commitment to patient-centered, value-based primary care

- Experience working in a team-based, technology-enabled care environment
- Comfort with a broad range of primary care conditions
