This role focuses on providing skilled nursing care in the home setting, including patient assessments, care planning, and ongoing clinical monitoring. Key Responsibilities
- Conduct home-based nursing assessments and ongoing patient monitoring
- Provide skilled nursing care and education to patients and families
- Coordinate care plans with the interdisciplinary care team
- Document clinical findings, interventions, and patient progress
- Monitor changes in condition and communicate updates to the provider team
Required
- Active Registered Nurse (RN) license
- Relevant nursing experience in a clinical care setting
