Key Responsibilities
- Provide primary care across preventive health, chronic disease management, urgent care, and pediatrics.
- Deliver care in the clinic and through digital channels as a key primary care provider for the patient population.
- Collaborate with behavioral health, virtual urgent care and triage, and clinical subspecialists.
- Educate patients about their health conditions and develop individualized care and treatment plans.
- Use data, technology, and operational rigor to support patient outcomes.
- Address patient needs efficiently by partnering with team members and using a resourceful, collaborative approach.
Required
- MD or DO with training in Family Medicine.
- Active board certification in Family Medicine.
- Active Texas medical licensure or ability to obtain licensure by the start date.
- At least 5 years of clinical experience in primary care and chronic care for all ages.
- Comfort providing care for pediatric patients.
- Comfort with basic procedural skills.
Preferred
- Bilingual in English and Spanish.
- Strong communication skills with the ability to translate complex ideas into simple language.
