Provide synchronous telehealth visits for patients with cardiometabolic conditions, including diabetes, hypertension, obesity, and hyperlipidemia. The work centers on real-time medical consultations, with assessment, treatment recommendations, prescribing, and referral decisions made within the scope of the scheduled visit.
Key Responsibilities
- Conduct synchronous video consultations for patients with cardiometabolic conditions.
- Perform focused medical assessments, review histories, and address concerns during the encounter.
- Provide medical recommendations, prescriptions, or referrals in accordance with state licensure and standards of care.
- Document patient encounters accurately in the electronic health record.
- Work within a virtual specialty care model focused on chronic disease management.
Required
- Medical degree (MD or DO) from an accredited institution.
- Active board certification in Internal Medicine, Family Medicine, Endocrinology, Cardiology, Obesity Medicine, or a related specialty.
- Experience treating cardiometabolic conditions such as type 2 diabetes, hypertension, hyperlipidemia, and obesity.
- Active medical license in the state(s) where services are provided.
- Ability to deliver care virtually with strong communication skills.

- Prior telemedicine experience.
