Provide virtual nursing support for members living with chronic cardiometabolic conditions, including obesity, diabetes, hypertension, and dyslipidemia. The role centers on telehealth visits, messaging-based triage, member education, and escalation of concerns to the appropriate clinical provider. It is an independent contractor engagement within a tech-enabled virtual care model.
Key Responsibilities
- Conduct scheduled telehealth nursing visits with structured intake, assessment, medication reconciliation, and symptom review.
- Provide messaging-based support for member questions about symptoms, medications, side effects, and chronic disease management.
- Deliver evidence-based education on medication use, side-effect management, lifestyle guidance, and self-management.
- Identify clinical concerns, medication issues, or changes in status that require escalation to a provider or care team.
- Document assessments, education, escalations, and follow-up needs accurately and timely in the electronic health record.
- Coordinate follow-up information to support continuity of care within established protocols and scope of practice.
Required
- Active, unrestricted Registered Nurse license in good standing.
- BSN required.
- Ability and willingness to obtain and maintain additional state licenses as needed for contracted services.
- At least 5 years of clinical nursing experience.
- At least 1 year of experience in telehealth, virtual care, or ambulatory care.
Preferred
- CDCES or another relevant clinical certification.
- Experience supporting patients with diabetes, obesity, hypertension, dyslipidemia, or other cardiometabolic conditions.
- Experience working independently with virtual care tools and electronic health records.
